WHO I AM This website is operated by Templar Bacha ("I," "me," or "my"), who provides independent WorkSafeBC claim review services to injured workers in British Columbia. I am not affiliated with or endorsed by WorkSafeBC. I am committed to protecting your personal information and handling it in accordance with British Columbia's Personal Information Protection Act ("PIPA"). This policy explains what personal information I collect, why I collect it, how I use and protect it, and your rights. 2. WHAT PERSONAL INFORMATION I COLLECT Depending on how you interact with me, I may collect: - Contact information, such as your name, email address, and phone number. - Basic claim details you provide through the contact form, such as your claim number, the date of your decision letter, and a brief description of the decision. - Claim file information, if you become a client, including decision letters, correspondence with WorkSafeBC, employment and earnings information, and medical information and reports contained in your claim file. - Information you share during consultations, calls, and meetings. - Payment and billing information needed to process fees. HOW I COLLECT PERSONAL INFORMATION I collect personal information directly from you, through the contact form on this website, by email, by phone or video call, and through documents you choose to share with me. With your consent, I may also receive information from others involved in your claim, such as a representative you authorize. Please do not send documents, medical records, or detailed health information through the website contact form. Once we have spoken, I will let you know how to share your file securely. 4. WHY I COLLECT PERSONAL INFORMATION I use your personal information to: - Respond to your inquiry and schedule consultations. - Confirm whether I am able to assist you, including checking for conflicts of interest. - Review your WorkSafeBC decision and claim file and prepare written reports. - Assist with drafting or editing a request for review, if you engage me to do so. - Communicate with you about your matter. - Issue invoices and process payments. - Meet my legal, regulatory, and professional obligations. I will not use your personal information for any other purpose without your consent, unless permitted or required by law. 5. CONSENT By submitting the contact form or engaging my services, you consent to the collection, use, and disclosure of your personal information as described in this policy. Where information is particularly sensitive, such as medical information, I will obtain your express consent before collecting it. You may withdraw your consent at any time by contacting me, subject to legal or contractual restrictions and reasonable notice. If you withdraw consent, I may be unable to continue providing services, and I will explain the likely consequences to you. 6. HOW I SHARE PERSONAL INFORMATION I do not sell, rent, or trade your personal information. I share personal information only: - With your consent, for example with WorkSafeBC or a representative you authorize. - With service providers who help me operate my services, including GoDaddy (website hosting and contact form) and Google (email and file storage). These providers may only use your information to provide services to me. - When required or authorized by law. My service providers, including GoDaddy and Google, may store information on servers located outside of Canada, including in the United States. Information stored outside Canada may be subject to the laws of that jurisdiction. 7. HOW I PROTECT PERSONAL INFORMATION I use reasonable security measures to protect your personal information against unauthorized access, collection, use, disclosure, copying, modification, or disposal. These include password-protected accounts with two-factor authentication, secure file-sharing methods for documents, restricting access to your information to myself and service providers who need it, and secure deletion of records that are no longer required. No method of transmitting or storing information is completely secure. If you have concerns about sending information to me by email, please contact me to discuss alternatives. 8. HOW LONG I KEEP PERSONAL INFORMATION I keep personal information only as long as necessary to fulfil the purposes for which it was collected, and as required by law or professional obligations. Where I use your personal information to make a decision that directly affects you, I will keep it for at least one year after that decision, as required by PIPA. Records that are no longer needed are securely deleted or destroyed. Inquiries that do not result in an engagement are deleted within [12 months] unless you ask me to keep them. 9. WEBSITE, COOKIES, AND ANALYTICS This website is hosted by GoDaddy. Like most websites, it may use cookies and similar technologies to operate properly and to collect general information about how visitors use the site, such as pages visited and approximate location. This information is used to improve the website and is not used to identify you personally. You can disable cookies through your browser settings, though some features may not work properly. This website may contain links to other websites, such as WorkSafeBC. I am not responsible for the privacy practices of other websites. 10.
